YEAR 4 SWIMMING CONSENT

Swimming Lessons at White Oak from Wednesday 23rd April onwards

I agree to my child's participation in swimming lessons as per the details in the letter I received. I agree with my child's participation in the swimming lesson and acknowledge the need for them to behave responsibly.

This form is valid for the said school lessons from the date you complete it, for the period of time your child attends the lessons. The consent will automatically expire after this time. We hold your records securely until the lessons have ended and all the children are returned to school, after which they are retained securely for 1 month in case of any queries, then safely destroyed.
